Os oes gennych unrhyw ofynion arbennig, rhowch wybod i ni a byddwn, lle bo modd, yn gwneud unrhyw addasiad rhesymol er mwyn cynorthwyo. Os oes angen dogfennau arnoch mewn unrhyw fformat arbennig, rhowch wybod i ni ac fe wnawn ein gorau i helpu. Mae rhan fwyaf o’r staff yn siarad Cymraeg. Disabled Access The practice premises provide suitable access for our patients. The whole premise is on ground level with no steps or stairs. There is a disabled toilet near the reception. Guide Dogs Guide dogs and service dogs are welcome. We have a mobile hearing loop on site which can be used during consultation. Please inform the Reception Team if you would like to use this facility. Equality The practice is committed to equality and will not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or a medical condition. If you have any special requirements, please inform us and we will, where possible, make any reasonable adjustment in order to assist. If you require documents in any special formats, please let us know and we will do our best to assist. Most of our staff are Welsh Speaking. We are always happy to help and are available to answer any questions or queries you may have.

Sylwch: Mae'n bwysig eich bod yn rhoi gwybod i'r staff cyn gynted â phosibl cyn eich apwyntiad, pa wasanaethau cyfieithu sydd eu hangen arnoch, yna gellir archebu'r rhain ymlaen llaw. Gwasanaeth ar gael Mae amrywiaeth o opsiynau ar gael ar gyfer cael gwasanaethau dehongli; rhestrir y rhain isod. Gan ddibynnu ar eich amgylchiadau a’r wybodaeth sy’n cael ei chyfleu, efallai y bydd angen defnyddio un opsiwn neu gyfuniad o’r opsiynau sydd ar gael: • Gwasanaethau Dehongli Ffôn – gellir eu cynnal ar adeg yr apwyntiad ym mhob iaith. Gellir darparu cyfieithwyr Cymraeg ar gyfer eich apwyntiad. Darperir y gwasanaeth gan Wasanaeth Dehongli a Chyfieithu Cymru (WITS) sy'n dod o hyd i ddehonglwyr a chyfieithwyr proffesiynol ac yn eu dyrannu i'w cymhwyso yn y sector cyhoeddus https://www.wits.wales
If you are a patient that does not communicate comfortably in English or Welsh, or are deaf or hard of hearing we can arrange an interpretation service for you. Please note: It is important that you let the staff know as soon as possible before your appointment, which interpretation services you require, these can then be booked in advance. Service available There are a variety of options available for obtaining interpretation services; these are listed below. Depending on your circumstances and information being conveyed, either a single or a combination of available options may need to be used: • Telephone Interpretation Services – can be carried out at the time of appointment in all languages. This type of service is useful when conveying short or routine medical information. This Service is a three-way telephone conference facility which provides almost immediate access to skilled, trained registered interpreters. • Welsh Language Translation – If Welsh is your first Language and you feel more comfortable communicating in the medium of Welsh. Welsh translators can be provided for your appointment. The service is provided by The Wales Interpretation and Translation Service (WITS) which sources and allocates professional interpreters and translators for public sector application https://www.wits.wales/
